Adoption and Surrogacy

St. Luke’s is dedicated to supporting all parties involved in an infant adoption or surrogacy arrangement with compassion and sensitivity. Our providers, nurses, and staff are trained in the complexity of adoption and surrogacy and are committed to respecting the unique needs of intended parents, prospective parents, surrogates, gestational carriers, and the newborns who connect them. 

We know this life-changing event is experienced differently by each person who goes through it. We are here for each of you—adjusting our care to support your needs and providing a safe environment for you to heal.

What to Expect – Surrogacy or Birth Patient

  • Expert care during childbirth by staff trained in adoption and surrogacy
  • A specific birthing plan designed for you
  • A commitment to do all we can to best meet your individual needs 
  • Understanding, compassion, and support 
  • Grief and loss counseling referrals, if needed

What to Expect – Intended and Prospective Parents

  • The same education we share with all our families who have a new baby
  • Support to help you gain confidence in your ability to care for your new baby
  • A specific birthing plan designed for you
  • Registration, medical records, and billing that reflect your unique needs
  • Room-in with your baby at no cost, when available
  • A commitment to do all we can to best meet your individual needs
  • Understanding, compassion, and support

Forms to be Completed With Your Attorney or Agency

Adoption

Please complete the following two forms and return to Patient Financial Services as soon as possible via email or fax to 208-706-7619.

  • Financial Form for Adoption - English | Spanish 
  • Authorization to Access, Use, and Disclose Protected Health Information (PHI) - English | Spanish - you will need to complete two of these forms: one for the newborn and one for the birth patient

And, return the following forms to our delivery support team via email as soon as possible: 
Email for Boise, McCall, Meridian and Nampa deliveries | Email for Magic Valley and Wood River deliveries

  • Power of Attorney from your attorney’s office, if available
  • Authorization for Visitation of Infant - English | Spanish
  • Plan of Care: Adoption - English | Spanish
  • Delegation of Authority to Make Health Care Decisions (Idaho) - English | Spanish

Surrogate, or Gestational Carrier WITHOUT a Court-Validated Gestational Agreement

Please complete the following forms and return to Patient Financial Services by 13 weeks gestation via email or fax to 208-706-7619.

  • Financial Form for Surrogacy - English | Spanish
  • Authorization to Access, Use, and Disclose Protected Health Information (PHI) - English | Spanish - you will need to complete two of these forms: one for the newborn and one for the birth patient

And, return the following forms to our delivery support team via email as soon as possible:

Special Circumstances

There are occasionally special circumstances that also require the pre-submission of forms (e.g., patients living in a correctional facility). If you have concerns that this may apply to you, please send us an email.
